Most cited CFR sections
All violations issued in the selected period. Counts combine subsections.
EE reports sudden upper abdomen pain, while pulling a water hose, during the process of unchoking clay in the discharge chute of the Rod Mill. EE evaluated by on-site Paramedic and then made the decision to go to the ER for evaluation of the injury.
During a nighttime anchor move, the employee missed a step and fell on the excavator barge deck. Humidity and mud spots were present; slip-resistant boots were worn. Employee landed on the gluteal area and noted mild left lower-back discomfort that progressively worsened, affecting bending, lifting, sitting, standing, and stretching. Reported late on 9/2/26 following rotation.
EE reports stepping down from water truck and slipping in mud. Upon slipping EE turned to grab handrail to prevent fall to ground, at this motion EE experienced a twisting and pulling sensation in lower back. This caused a sharp pain in the left lower back. EE was sent to the Clinic for evaluation.
Employee was walking in a work area and suddenly felt like EE was being bit. The employee removed EE's shirt and had other employees examine EE for bites. The employee described feeling stinging. The employee did not report the incident until 7/28/2026 by way of texting after not showing up for work.
EE reports backing over a drop-off and experiencing neck pain as a result of the whipping motion that it caused. EE went to ER after incident and was contacted by Clinic the next morning to obtain documentation for the incident.
HANDLING OF MATERIALS · DAYS RESTRICTED ACTIVITY ONLY
Accident narrative
EE states something popped in left upper arm while working to rotate a crimper. EE reports mild discomfort in the area. On exam there is a notable bulge in the bicep muscle in the middle of the upper arm.
Counts describe published rows, including contractor records; accident records are not distinct incidents or numbers of people injured.
Zero means no matching records in the imported sources, not proof of no events. Historical coverage differs by source; absence before a source begins reporting is not a zero incident rate. Invalid or absent dates and records after the snapshot are excluded.
S&S means SIG_SUB=Y. Fatality records use DEGREE_INJURY_CD=01. Proposed penalties are not final assessed or paid amounts; missing dollar values are omitted.
Inspection hours are SUM_TOTAL_ON_SITE_HOURS attributed to the inspection start date, without supervisor/trainee hours. Missing hours are omitted; a started inspection may still be open.
Portfolio totals use current mine membership, including events before the current owner or operator's tenure.
CFR headings link to the current Title 30 index; they are not historical legal determinations.
