The air in the longwall headgate #2 entry inby of the longwall face had an oxygen content of less that 19.5% and was continuing to drop. The crew was pulled from the area until the ventilation could be fixed and the oxygen content was restored to above 19.5%.
While putting a drilling machine on the cage to be hoisted out the cage faults causing it to drop roughly 3 feet until the safety system set the brakes.
A roof fall occurred in the tailgate bleeder entry of the longwall. The tailgate entry was cut off sites causing the shear to extend into the entry further than normal. This causing the shear to cut out the crib in the tailgate.
While supervisor was going in to preshift the area of the initial K order so the crew could go in and start working on mitigating the fall that occurred on 9-4-2026 they found that another area had fallen. A plan has been submitted to MSHA and rehabilitation work is ongoing.
A roof fall occurred in the tailgate bleeder entry of the longwall that was above anchor point. The fall was not in an active working area, nobody was injured, and it did not affect ventilation. While inspecting the return a roof fall was found in 1 entry at 0 cross cut of the 458 North area. The area was flagged off, no injuries reported, and ventilation was not disrupted.
Counts describe published rows, including contractor records; accident records are not distinct incidents or numbers of people injured.
Zero means no matching records in the imported sources, not proof of no events. Historical coverage differs by source; absence before a source begins reporting is not a zero incident rate. Invalid or absent dates and records after the snapshot are excluded.
S&S means SIG_SUB=Y. Fatality records use DEGREE_INJURY_CD=01. Proposed penalties are not final assessed or paid amounts; missing dollar values are omitted.
Inspection hours are SUM_TOTAL_ON_SITE_HOURS attributed to the inspection start date, without supervisor/trainee hours. Missing hours are omitted; a started inspection may still be open.
Portfolio totals use current mine membership, including events before the current owner or operator's tenure.
CFR headings link to the current Title 30 index; they are not historical legal determinations.
