Most cited CFR sections
All violations issued in the selected period. Counts combine subsections.
The employee was working on the board rock hopper motor removing old bolts and brackets. EE was standing in a position with limited mobility swinging a sledgehammer in an upwards motion. The employee felt a sharp pain in their right arm, stopped and noticed some bruising. The employee was taken to the company doctor and treated for a bicep strain.
The hoist was in operation bringing dumpsters to the surface from the mining level. The hoist operator stopped the cage at the mining level to change dumpsters and the hoist tripped out, saying "communication failure". After failing to restart the hoist the electricians were notified. The hoist was down for longer than 30 minutes so MSHA was notified. Minimal damage and impact.
Hoist operator heard a buzzing noise coming from the breaker area for the main hoist. EE had an electrician look into it and said a coil could be getting ready to fail. As a precaution we took the main hoist out of service for diagnosis and/or repair. Miners evacuated out the escape shaft. No miners were injured.
Counts describe published rows, including contractor records; accident records are not distinct incidents or numbers of people injured.
Zero means no matching records in the imported sources, not proof of no events. Historical coverage differs by source; absence before a source begins reporting is not a zero incident rate. Invalid or absent dates and records after the snapshot are excluded.
S&S means SIG_SUB=Y. Fatality records use DEGREE_INJURY_CD=01. Proposed penalties are not final assessed or paid amounts; missing dollar values are omitted.
Inspection hours are SUM_TOTAL_ON_SITE_HOURS attributed to the inspection start date, without supervisor/trainee hours. Missing hours are omitted; a started inspection may still be open.
Portfolio totals use current mine membership, including events before the current owner or operator's tenure.
CFR headings link to the current Title 30 index; they are not historical legal determinations.
