MACHINERY · DAYS AWAY FROM WORK ONLY
A miner was finishing driving a split set bolt and as EE was removing the jack leg the driver hung up in the bolt. When pulling on it the driver freed suddenly causing EE to step back awkwardly on a rock resulting in a right knee injury. MRI showed a ruptured patella tendon.

HOISTING · ACCIDENT ONLY
The Coeur hoist went down due to a broken secondary connector. The electrician found the problem and replace it. These are new connectors and don't appear to hold up as well. Hoist was back up at 8:50 am. The connectors will be checked weekly.
HOISTING · ACCIDENT ONLY
At 10:37 pm the chippy hoist experienced a power surge that tripped the main breaker. The hoist operator is fairly new and wasn't confident in resetting the breaker, so EE called electricians. While they were in travel, we stood down the crews and prepared to hoist them to 3700. The electricians arrived, reset the breaker and the hoist was back to duty at 11:30pm.

MSHA Violations (issue date), Accidents (accident date), and Inspections (begin date). Snapshot 2026-10-18.
Counts describe published rows, including contractor records; accident records are not distinct incidents or numbers of people injured.
Zero means no matching records in the imported sources, not proof of no events. Historical coverage differs by source; absence before a source begins reporting is not a zero incident rate. Invalid or absent dates and records after the snapshot are excluded.
S&S means SIG_SUB=Y. Fatality records use DEGREE_INJURY_CD=01. Proposed penalties are not final assessed or paid amounts; missing dollar values are omitted.
Inspection hours are SUM_TOTAL_ON_SITE_HOURS attributed to the inspection start date, without supervisor/trainee hours. Missing hours are omitted; a started inspection may still be open.
Portfolio totals use current mine membership, including events before the current owner or operator's tenure.
CFR headings link to the current Title 30 index; they are not historical legal determinations.
