Most cited CFR sections
All violations issued in the selected period. Counts combine subsections.
Employee was changing a bottom-top screen in the mill. After completing the screen change the employee went to step out of the screen and back on to the walkway. When EE went to step on to the walkway, EE did not plant EE's foot correctly. Foot/ankle twisted which resulted in pain to EE's left foot due to landing on it incorrectly.
The employee was working on a metal skirting in building #2, which at some point was attached to the crane, as the crane was being used to lift/move the skirting it sprang loose and struck the employee on the knee causing them to lose their balance and hit the middle of their back against the chute.
SLIP OR FALL OF PERSON · DAYS RESTRICTED ACTIVITY ONLY
Accident narrative
EE was going to sit in a chair in the dayroom prior to our kickoff meeting at 5am. Another employee then proceeded to pull EE's chair out from underneath EE causing EE to fall. As EE fell, EE turned to EE's left to avoid falling on EE's back or hitting EE's head. EE fell on EE's left arm (elbow and shoulder).
Counts describe published rows, including contractor records; accident records are not distinct incidents or numbers of people injured.
Zero means no matching records in the imported sources, not proof of no events. Historical coverage differs by source; absence before a source begins reporting is not a zero incident rate. Invalid or absent dates and records after the snapshot are excluded.
S&S means SIG_SUB=Y. Fatality records use DEGREE_INJURY_CD=01. Proposed penalties are not final assessed or paid amounts; missing dollar values are omitted.
Inspection hours are SUM_TOTAL_ON_SITE_HOURS attributed to the inspection start date, without supervisor/trainee hours. Missing hours are omitted; a started inspection may still be open.
Portfolio totals use current mine membership, including events before the current owner or operator's tenure.
CFR headings link to the current Title 30 index; they are not historical legal determinations.
