Most cited CFR sections
All violations issued in the selected period. Counts combine subsections.
Still under investigation, as we believe it was an electrical fire that starter on our Kleemann crusher over night which spread to our conveyor and other crusher and screen. It was discovered that morning as employees was coming to work there was a fire in the mine and they called 911 and myself to get everyone notified: MSHA, ODNR and chain of command
Conveyor went down due to being overloaded and they commenced to clearing the belt off to get back running when employee was repositioning feet they caused them to fall down on the rock surface below conveyor landing on their back onto the loose rocks. CT scans and x-rays determined they had compound fractures in their T1, T11 and T12 discs in the back along with a bruised lung.
EE was holding a metal pipe for another coworker to weld on a stand pipe. EE was wearing tinted safety glasses while holding the pipe and said while turning EE's head the other Co-worker struck the arc to do the tack weld the arc caught the corner of EE's eye. EE had redness in EE's eye from the arc flash and needed medical attention.
Counts describe published rows, including contractor records; accident records are not distinct incidents or numbers of people injured.
Zero means no matching records in the imported sources, not proof of no events. Historical coverage differs by source; absence before a source begins reporting is not a zero incident rate. Invalid or absent dates and records after the snapshot are excluded.
S&S means SIG_SUB=Y. Fatality records use DEGREE_INJURY_CD=01. Proposed penalties are not final assessed or paid amounts; missing dollar values are omitted.
Inspection hours are SUM_TOTAL_ON_SITE_HOURS attributed to the inspection start date, without supervisor/trainee hours. Missing hours are omitted; a started inspection may still be open.
Portfolio totals use current mine membership, including events before the current owner or operator's tenure.
CFR headings link to the current Title 30 index; they are not historical legal determinations.
