INCIDENT HAPPENED 08/13/2025 NEVER PLACED ON LIGHT DUTY. RECEIVED PHYSICAL THERAPY-FULL DUTY. PRESCRIPTION ISSUED ON 11/06/2025. FULL DUTY. EMPLOYEE WAS HELPING PACK A PALLET WITH TWO SMALL CYLINDERS, TRIPPED OVER A PALLET, AND FELL ON TRUNK. PALLET FELL TO THE GROUND. THE ZIP TIE HOLDING THE CYLINDERS ON THE PALLET BROKE, ONE ROLLED OFF ON EMPLOYEES R. ANKLE AND LOWER LEG.
