Accident narrative
Lost power to all surface and underground operations, which caused us to lose hoisting capabilities. Issue was within the city of Cleveland. Notified Cleveland Public Power (CPP) of issue and proceeded to evacuate mine until power was restored.

The brake sensor on the Phoist malfunctioned. Upon investigation, we found that a wire was loose/disconnected and needed to be corrected.
