Accident narrative
Two employees were reinstalling guards and cleaning around the 1�-inch feed-belt tail pulley. While exiting the area with a pry bar, one employee stepped into wet accumulated material, causing EE's foot to sink. As EE pulled free, EE struck EE's forehead on a fixed crossbar. EE sustained a laceration above EE's right eyebrow and received nine stitches.
