All violations issued in the selected period. Counts combine subsections.
STEPPING OR KNEELING ON OBJECT · DAYS AWAY FROM WORK ONLY
Accident narrative
Employee had just completed cleaning off the tracks and was walking back to EE's work area when EE stepped on a rock and twisted EE's left ankle. EE's foot turned one way, and the momentum caused EE's body to turn the other way, which then caused EE's ankle to become dislocated and fracture.
HANDLING OF MATERIALS · DAYS RESTRICTED ACTIVITY ONLY
Accident narrative
An employee sustained a laceration to the left pinkie while closing the front gate. The injury caused significant bleeding. They were transported to urgent care where they were referred to the ER. The finger was fractured in two places and required seven stitches. A follow-up with a hand specialist is scheduled.
A 50 lb. bagger employee was opening an air valve at silo #2 in an attempt to unstop the line around 6:00 pm. The employee struggled to open the valve and felt that EE had pulled a muscle, so EE stopped work and reported the incident.
HANDTOOLS (NONPOWERED) · NO DYS AWY FRM WRK,NO RSTR ACT
Accident narrative
Maintenance technician was insulating piping, EE was holding a water line while cutting the insulation to fit. The pipe shifted in the middle of EE's cut, and EE cut EE's left pinkie finger. EE was taken to local urgent care where EE received 4 stitches. EE was then released to work with no restrictions.
Counts describe published rows, including contractor records; accident records are not distinct incidents or numbers of people injured.
Zero means no matching records in the imported sources, not proof of no events. Historical coverage differs by source; absence before a source begins reporting is not a zero incident rate. Invalid or absent dates and records after the snapshot are excluded.
S&S means SIG_SUB=Y. Fatality records use DEGREE_INJURY_CD=01. Proposed penalties are not final assessed or paid amounts; missing dollar values are omitted.
Inspection hours are SUM_TOTAL_ON_SITE_HOURS attributed to the inspection start date, without supervisor/trainee hours. Missing hours are omitted; a started inspection may still be open.
Portfolio totals use current mine membership, including events before the current owner or operator's tenure.
CFR headings link to the current Title 30 index; they are not historical legal determinations.
