Accident narrative
An employee was attempting to remove a sharp metal shaving from a pipe that was threaded using a pipe threader machine. When they pulled on the metal shaving, it did not separate from the pipe as expected, causing the sharp edge to lacerate their finger through their glove. The injured employee received 8 stitches to their index finger.

Accident narrative
Dozer abruptly stopped on its own causing the operator's upper body to move in a forward while the lap belt held the lower body in the seat. The operator then experienced pain in back with numbness and tingling. Contributing fact was a loose wire at the bottom of the connector and an internal locking tab failure within the connector, allowing the wire to back out.
SLIP OR FALL OF PERSON · DAYS RESTRICTED ACTIVITY ONLY
Accident narrative
Employee was climbing off the ladder of a 993 loader. The employee missed the last step causing EE to fall to the ground injuring EE's right elbow.
SLIP OR FALL OF PERSON · DAYS RESTRICTED ACTIVITY ONLY
Accident narrative
Emloyee stepped out of a medium duty vehicle onto uneven ground and rolled their ankle. This caused a fracture to the distal fubula.
SLIP OR FALL OF PERSON · DAYS AWAY FROM WORK ONLY
Accident narrative
Employee was climbing down the steps of a blast hole drill, when the employees first foot touched the ground EE let go of the handholds with EE's hands, at this moment the other foot got caught on the last rung of the steps causing EE to fall backward onto the ground. The employee had soreness and stiffness in the lower right back.
SLIP OR FALL OF PERSON · DAYS RESTRICTED ACTIVITY ONLY

Accident narrative
This injury was classified as a first aid and then later diagnosed on June 4, 2025, as a possible meniscus tear, and will need to be scoped to identify the injury.
SLIP OR FALL OF PERSON · DAYS RESTRICTED ACTIVITY ONLY
Accident narrative
The employee stated they stepped off a haul truck ladder to the ground and felt their knee buckle backward. The employee had ample time in the shift to report the injury to the supervisor and did not. We are unsure of the actual location that the injury occurred.
SLIP OR FALL OF PERSON · DAYS RESTRICTED ACTIVITY ONLY
Accident narrative
EE stepped on grating with accumulated mud on soles of their boots resulting in spraining left ankle.
MACHINERY · DAYS RESTRICTED ACTIVITY ONLY
Accident narrative
This incident was a first aid that escalated into medication that was prescribed for inflammation and pain on 3/18/25. The operator was training in a shovel when they fell against a box on the wall hitting their back impacting ribs area.

This incident was under investigation for happening on mine site & with no diagnosis until further medical imaging is required. Waiting for exact diagnosis treated as first aid until further detail
Safety sources and definitions
MSHA Violations (issue date), Accidents (accident date), and Inspections (begin date). Snapshot 2026-10-18.
Counts describe published rows, including contractor records; accident records are not distinct incidents or numbers of people injured.
Zero means no matching records in the imported sources, not proof of no events. Historical coverage differs by source; absence before a source begins reporting is not a zero incident rate. Invalid or absent dates and records after the snapshot are excluded.
S&S means SIG_SUB=Y. Fatality records use DEGREE_INJURY_CD=01. Proposed penalties are not final assessed or paid amounts; missing dollar values are omitted.
Inspection hours are SUM_TOTAL_ON_SITE_HOURS attributed to the inspection start date, without supervisor/trainee hours. Missing hours are omitted; a started inspection may still be open.
Portfolio totals use current mine membership, including events before the current owner or operator's tenure.
CFR headings link to the current Title 30 index; they are not historical legal determinations.

Silica and quartz
Coal quartz, metal/nonmetal personal samples and area samples. Snapshot Oct 18, 2026. 16 silica records across the mine’s published history.
Personal silica samples · concentration / limit
Quarterly mean · published C/E ratio
— No eligible measurements. * Quarter in progress. Means describe the available samples, not average exposure across the mine.
Quartz % is the sample’s quartz content; µg/m³ is its reported concentration. Personal C/E is the ratio to the limit published with that sample. Personal and area concentration units and void status are not specified in those source files.
| Date / sample | Dataset / analyte | Occupation / area | Measurement | Sample status | Published action |
|---|---|---|---|---|---|
| No silica records match these filters. Missing samples do not establish safe conditions. | |||||
Sources and interpretation
- Coal means use numeric concentrations from samples marked not voided, excluding control/invalid sample types and dates after the snapshot. Missing values remain null; reported zeroes remain zero.
- Quarterly means describe available samples, not mine-wide or worker-average exposure. Quarters with no eligible measurements remain empty; the current quarter is partial.
- Personal C/E is MSHA's published concentration-to-limit ratio. A value above 1 is not by itself a citation or a determination under the 2024 silica rule. Personal and area files do not publish a void-status field.
- Area records include bulk-dust analyses and are kept separate from personal exposure results. Their concentration units are not specified in the source dictionary.
- Missing samples do not establish safe conditions. Coal quartz percentage and concentration are separate measures. No compliance threshold is inferred for coal samples.

Limitations
- Contractor production is entity-level and is never represented as mine-attributable.
- Missing rows can mean no event, publication delay, identity mismatch, or inapplicability.
