SLIP OR FALL OF PERSON · DAYS AWAY FROM WORK ONLY
Accident narrative
Employee tripped while walking between the motor and handrail to assist a coworker and fell onto the grating, resulting in a pinky finger injury.
SLIP OR FALL OF PERSON · DYS AWY FRM WRK & RESTRCTD ACT
Accident narrative
Employee was backing up pulling vacuum pipe out of sump and EE's foot got caught on vacuum breaker causing EE to loose EE's balance. EE rotated and landed on EE's right knee as EE was trying to catch self.
SLIP OR FALL OF PERSON · ALL OTHER CASES (INCL 1ST AID)
Accident narrative
Individual attempted to dismount forklift via side steps, overstepped the last step causing to fall backwards. Individual attempted to stop impact from fall by extending arm. Left wrist was sprained in the process. Individual was seen by onsite medical team and refused further treatment, as EE didn't feel it was necessary.
Impoundment Spot Inspection · 4.0 on-site hours
HANDTOOLS (NONPOWERED) · DAYS AWAY FROM WORK ONLY
Accident narrative
While removing a bed pin, stored energy in the component caused it to suddenly dislodge and shift the bed. The uncontrolled movement allowed the brass bar to slip and drop, pinning the employee's hand against the tire. Resulted in a hand injury from impact/pinch force.
POWERED HAULAGE · DAYS AWAY FROM WORK ONLY
Accident narrative
Loose material during loading allowed a rock to roll from the bucket and strike Cat 75, causing a rough load event. The impact broke the rear window. The operator later developed back pain and required clinic evaluation by ERT.
103 (g) Written Notification Hazard Complaint Inspectio · 6.5 on-site hours
SLIP OR FALL OF PERSON · DAYS RESTRICTED ACTIVITY ONLY
Accident narrative
The employee met with supervisors in a parking lot on the east side of the building. After the conversation, EE turned to walk back to the office and tripped over a wheel chock (angle iron) because EE did not fully clear it. EE fell onto EE's right knee and elbow. EE was wearing steel-toe boots, and no other PPE was involved.
103 (g) Written Notification Hazard Complaint Inspectio · 5.0 on-site hours
HANDLING OF MATERIALS · DAYS AWAY FROM WORK ONLY
Accident narrative
Employee felt a groin strain while cleaning but delayed reporting it until the end of the shift when the pain persisted.
STRIKING OR BUMPING · NO DYS AWY FRM WRK,NO RSTR ACT
Accident narrative
Employee inspecting a hose stood up in a confined space and struck their hard hat on a frame causing their jaw to snap shut and chip a tooth with a minor gum laceration.
HANDLING OF MATERIALS · DAYS AWAY FROM WORK ONLY
Accident narrative
Employee used Brakleen to remove hydraulic oil from their hands; after doing so they took a smoke break causing burns to their hands.
HANDLING OF MATERIALS · DAYS AWAY FROM WORK ONLY
Accident narrative
The operator was dismounting equipment using a ladder when they experienced pain and noted swelling in the knee.
HANDLING OF MATERIALS · DAYS RESTRICTED ACTIVITY ONLY
Accident narrative
While cleaning a flowmeter for install, employee cut right finger on the sensor requiring stitches.
HANDLING OF MATERIALS · DAYS RESTRICTED ACTIVITY ONLY
Accident narrative
While employee was removed tires from a connex for delivery, they noticed overlap of tires with an adjacent stack. As additional force was used to move the overlapped tire, the adjacent stack became unstable causing some tires to fall and strike the employee which resulted in the employee falling and straining their back.
Safety sources and definitions
MSHA Violations (issue date), Accidents (accident date), and Inspections (begin date). Snapshot 2026-10-18.
Counts describe published rows, including contractor records; accident records are not distinct incidents or numbers of people injured.
Zero means no matching records in the imported sources, not proof of no events. Historical coverage differs by source; absence before a source begins reporting is not a zero incident rate. Invalid or absent dates and records after the snapshot are excluded.
S&S means SIG_SUB=Y. Fatality records use DEGREE_INJURY_CD=01. Proposed penalties are not final assessed or paid amounts; missing dollar values are omitted.
Inspection hours are SUM_TOTAL_ON_SITE_HOURS attributed to the inspection start date, without supervisor/trainee hours. Missing hours are omitted; a started inspection may still be open.
Portfolio totals use current mine membership, including events before the current owner or operator's tenure.
CFR headings link to the current Title 30 index; they are not historical legal determinations.

Silica and quartz
Coal quartz, metal/nonmetal personal samples and area samples. Snapshot Oct 18, 2026. 100 silica records across the mine’s published history.
Personal silica samples · concentration / limit
Quarterly mean · published C/E ratio
— No eligible measurements. * Quarter in progress. Means describe the available samples, not average exposure across the mine.
Quartz % is the sample’s quartz content; µg/m³ is its reported concentration. Personal C/E is the ratio to the limit published with that sample. Personal and area concentration units and void status are not specified in those source files.

Sources and interpretation
- Coal means use numeric concentrations from samples marked not voided, excluding control/invalid sample types and dates after the snapshot. Missing values remain null; reported zeroes remain zero.
- Quarterly means describe available samples, not mine-wide or worker-average exposure. Quarters with no eligible measurements remain empty; the current quarter is partial.
- Personal C/E is MSHA's published concentration-to-limit ratio. A value above 1 is not by itself a citation or a determination under the 2024 silica rule. Personal and area files do not publish a void-status field.
- Area records include bulk-dust analyses and are kept separate from personal exposure results. Their concentration units are not specified in the source dictionary.
- Missing samples do not establish safe conditions. Coal quartz percentage and concentration are separate measures. No compliance threshold is inferred for coal samples.

NOTES
Limitations
- Contractor production is entity-level and is never represented as mine-attributable.
- Missing rows can mean no event, publication delay, identity mismatch, or inapplicability.
