HOISTING · ACCIDENT ONLY
Accident narrative
The east brake on the Production Hoist was recirculating all oil to the tank. Valve V 10 was not closing. Replaced the valve and found a lot of rubber shavings in port holes leading into the valve. Flushed out port holes and installed valve. Everything tested good.
