Accident narrative
An unintentional roof fall occurred the front end of the previously mined 12L section. This fall was in the #1 crosscut between #6 and #7 entry. The roof in this area previously identified as an area of concern and was supported and dangered off. It has been noted that a stream valley runs though this area and may have contributed to the fault. approx 30'by 13.5' by 7' high.

Accident narrative
EE was cutting a discarded piece of belt with a belt utility knife. EE was cutting away from self and snapped the blade off. A small piece of the broken blade was lodged in the belt and as EE's momentum and EE's right linear distal medium forearm was struck against causing a 2CM laceration.

Accident narrative
EE was operating a diesel motor, EE was the second motor following another motor with a flat car. The involved EE's motor was not attached to the other motor. EE stood up while tramming to see the distance between motors when EE's hard hit a low hanging bag in the roof mesh. This caused EE's hard hat to strike EE's nose resulting in a nasal bone fracture.
