    Accident narrative

    While working to reinstall a rebuilt alternator EE was below the machine to support an awkward cover while other team members were running cables through the cover for installation. During the process a loose fastener moved from its storage location. The fastener fell and struck EE in the mouth, breaking several front teeth.

    Accident narrative

    Water truck operator stated EE blacked out while traveling west from the loading area. The truck traveled over the berm, tipped onto its side, slid down the pond slope, and came to rest upside down in the water. The operator regained consciousness, exited the truck, swam to shore, and walked up the hill for assistance. EE was transported to the hospital.
