Accident narrative
Maintenance was responding to a call that a rock had been stuck in the sand hopper. Employee and another maintenance member responded to unclog the rock and one of the loader operators that was unaware that employee was in the hopper dumped sand in and covered them.

Accident narrative
The 3 team members were using the telehandler to pick up the block and chose to use a rope instead of the rated chains on site. The team member was wrapping the rope around the rebar eye in the concrete block and the telehandler forks and as the telehandler operator lifter the forks the team members left middle finger got caught in between the rope and the rebar.
