A roof fall above anchorage was found in the primary escapeway in the intersection at wall 35 on b mains. The area was mine in June of 2012. The area has been cribbed off and the escapeway has been rerouted

Individual was installing the roof bolt into the roof when the channel plate slid down the bolt and lacerated EE's forearm, resulting in sutures.

Accident narrative
While drilling a hole in roof for a bolt, employee went to drop the steel out of the hole and the steel got caught, when they pulled on the steel it dropped hitting the drill head and bounced toward them. The steel struck their upper lip causing a laceration, resulting in three sutures.
