Employees were moving a refrigerator and one employee hit their head on the refrigerator when they attempted to maneuver it. Event went reportable on 10/16/2026.

Employee sustained a laceration to the left index finger while operating a hole saw to cut hole in poly drum lid

While a haul truck was being positioned for loading, the shovel dipper dropped into the truck bed, causing the bed to rise and the front tires to lift approximately 3 feet before returning to the ground. The employee sustained a contusion to the right knee and pain to the right wrist due to the impact and jolt.

Employee was installing zip ties per their daily work when they experienced discomfort in their right wrist while performing the task. Event went reportable 10/14/2026.

Tech was working on loader and accessed the elevated area using a 6-foot ladder. After staging the manifold on a portion of the loader, tech began to descend the ladder when they slipped/fell to the floor. They landed on their feet but, during the fall, their hardhat came off and the manifold fell from the loader and struck tech on the head. Taken to emergency care.

Tech working on LDR5 (994K loader) preparing to install manifold in hydraulic tank accessed elevated area via 6-ft ladder. After staging manifold, tech slipped and fell ~5 ft while descending. Hardhat came off; manifold fell from loader and struck tech on the head, causing a laceration. Tech landed on their feet

While loading a haul truck, the shovel dipper dropped into the truck bed due to a brake lockup, causing a severe jolt that lifted the truck bed and resulted in neck and upper back strains to the haul truck operator.

While traveling down a haul road a Haul Truck Operator encountered a rough road causing the operator to be was jostled around. They stated that the event caused pain in their back. The event went reportable on 10/09/2026.

The employee was working as part of the drip crew, assisting in the middle of the field with straightening the drip lines. While an ATV was pulling the drip line across the field, the line caught the employee, causing EE to trip and fall to the ground

An employee was detailing a van and used a handle to pull themselves up into the vehicle. During this action, the employee felt a sharp pain in their right shoulder. Event went reportable on 10/14/2026.

While working on a trip system on a dipper, a Mechanic was struck on the left pinky by an object. The event went reportable on 10/08/2026.
